As part of SAC HC's continuing effort to improve the coaching provided in the Travel program, players and parents requested to complete questionnaires covering the soccer experience for the previous season.

The questionnaires are one of the methods that will helps SAC HC evaluate the coaching staff.

The questionnaires are completely confidential. Only the Technical Director, the Assistant Directors and the president of the Club will view the forms.  Feedback will be given to the coaches based on the evaluations. 

It is important that every player and parent take time now during the season to fill out the questionnaire.  A better picture of the coach's performance over the season will be painted if everyone responds.

Please print out a copy of the evaluation form, which covers the appropriate age and mail it to:

Forms need to be returned to the SAC HC office by 12/10.
(Forms returned after 12/1 will NOT be able to be included in the end of the year evaluation.)
